Job Description

Minnesota Mental Health Clinics is excited to welcome a new Case Manager to our Horizons Community Support Program. Case managers are advocates for their clients and provide them with information, assistance to access resources, problem solving, decision making and moral support to maintain independent living. You will also help clients create realistic goals and work towards them in a planful way. Our clients are referrals from Dakota County Social Services. Role responsibilities and duties include:

  • Connect clients with available Dakota County resources to enable their daily lives
  • Such as assistance with mental health support, education, community and health programs, financial security, county and state insurance, housing, medication checks and refills, food security, insurance paperwork/renewals, etc.
  • Provide ongoing support to clients to enable them to access the resources they need
  • Communicate with clients and other agents via email, phone, video chat
  • Conduct check in with clients both in person and over the phone/video chat
  • Complete paperwork and documentation with a focus on quality and compliance, in a timely manner, including required assessments every 6 months
  • Attend weekly staff meetings and supervision. Collaborate with team members and supervisor to share learnings, experience, and help each other overcome challenges
  • Learn about available resources for clients and how to access them. Be resourceful in finding out more information about resources and leveraging team’s knowledge
  • Develop a treatment and care plan for clients based on their needs
  • Coordinate client care with other programs, resource agents, necessary supports and services in the best interest of clients (e.g. family, school, care givers, doctors, therapists etc.)
  • Manage a caseload of 30 clients, including adults and/or adolescents

Requirements

  • Bachelor’s degree in human service field and 2000 hours of experience providing vocational, recreational, educational and/or outreach services to those with mental illness
  • Demonstrate team membership qualities by maintaining effective working relationships and providing consistent and dependable work effort in job functions
  • Cooperates and maintains a high level of inter-staff communication designed to facilitate positive teamwork and to envelope consistent and effective approaches to clients
  • Current driver’s license and vehicle insurance
  • Must pass criminal background check
  • Computer skills including using Microsoft Office products, email, calendar, etc.
  • Willingness to learn, research and educate yourself on new resources and how to access them for your clients

  • Observation and understanding of client’s readiness for treatment and tailoring treatment plan to fit client’s ability and needs. Evolving treatment plans based on client’s progress
  • Have a non-judgmental mindset in your communication and interactions with clients
  • Diligence about keeping track of deadlines, details and paperwork to ensure timely and accurate filing
  • Self-motivation with strong time-management and organizational skills to stay on top of client documentation and independently manage your work
  • Ability to ask for help when needed without ego interfering with good client care
  • Friendly and supportive to co-workers who have diverse backgrounds and experience
  • Strong verbal communication skills with clients, co-workers, providers, and anyone else you will need to collaborate with to deliver quality care
  • Good self-care and establishment of boundaries to ensure your ability to continue to help clients
