Minnesota Mental Health Clinics’ Horizons program provides services and essential support to community members experiencing mental health difficulties, helping them live productive and satisfying lives.

About the MMHC Horizons Program

Horizons is committed to shared leadership and to a model of empowerment that promotes individual responsibility and self sufficiency. We are dedicated to delivering mental health services that enhance the quality of life for people with psychiatric disabilities and to eliminating the stigma that hinders employment, housing, and social opportunities. It is our strong belief that with proper support and acceptance, mental health clients can and do recover.

Horizons Targeted Case Management

We provide contracted Case Management services for both Dakota County and Hennepin County Social Services. The Horizons Case Manager will provide information, referrals to services in the community, and advocacy in the context of a supportive long-term relationship. They also help members set realistic goals and work towards them in a planful way.

About the Program

Our mission is to empower people on their recovery journey through compassionate, strengths-based case management that connects them to the services, resources, and supports they need to live well in the community.

Case managers can assist individuals meet their goals in various ways including mental health service needs, social functioning, self care improvement, housing, employment, continuing education, and more.

We offer flexible accommodations when meeting with individuals such as community settings, in home, and in office.

Housing
Provides members with assistance in finding adequate housing, applying for housing subsidies, and/or in-home support to maintain community living.
Employment
Provides referrals to all aspects of assistance in obtaining and maintaining competitive employment and volunteer positions. Provides support through case management, housing support, and Drop-In Center.
Benefits Assistance
Provides help with obtaining various subsidies, financial aid, and entitlements.
Case Management
Provides contracted case management services for Dakota County and Hennepin County Community Services.

Who We Support

  • We currently work with Dakota & Hennepin County residents ages 18 years or older.
  • Individuals diagnosed with an SPMI of Schizoaffective d/o, Schizophrenia, Major Depressive d/o, Bipolar d/o, or Borderline Personality d/o.
  • Receive Medical Assistance (MA) or qualifying for an exception to being on MA.

Hennepin County

Hennepin County residents can connect with their MMHC provider on how to submit a referral for case management OR contact Hennepin County Front Door.

Horizons Community Support Program

Our Community Support Program is located at our Eagan MMHC office. This program helps adults living with mental health challenges build the skills, confidence, and support they need to live well within the community.

Horizons Drop-In Center is available for Dakota County residents. The hours are: Monday – Thursday, 10am-4pm

Please check the calendar for the in-person days and times.

Members gather to play pool, cards, do arts and crafts, attend support groups, socialize or plan the next offsite activity. All Horizons services are available on site during drop-in hours.

We currently serve adults 18 years of age or older with Severe and Persistent Mental Illnesses (SPMI). Everyone welcome!

Horizons Community Support Program

Our CSP offers a variety of groups such as Psych Education, Cooking Groups, Arts & Crafts, Healthy Minds, Friendship Club, and so much more! The Community Support Program offers a safe space for individuals to connect with their community and obtain support for their mental health. We also offer transportation to and from our CSP for Dakota County residents (weather dependent). Prior sign up for transportation is required. The CSP also offers a variety of community outings including grocery shopping, Twins Games, Farmers Markets, Dakota County Fair, and more! Prior sign up is required for community outings.

Education
Links members with community educational resources and provides informative groups in which members participate as well as facilitate.
Social Recreational
Provides a wide range of community events and drop-in activities that are chosen and planned by members.
Outreach
Provides support and assistance in reducing the social isolation of members.
Friendship Program
Links volunteers with isolated members of the community who are experiencing mental illness.
Transportation
Provides members in southern Dakota County transportation to Horizons and community activities organized by Horizons.
